Legacy Neuropsychology partners with psychologists, family medicine, internal medicine, sports medicine, geriatric medicine, and neurology providers to support patients facing memory, cognitive, or capacity-related concerns. We aim to make the referral process as simple as possible, so your patients can be seen quickly, and your office spends as little time as possible on paperwork.

Insurance and Payment

Legacy Neuropsychology accepts Medicare. We do not participate with any other insurance plans. For patients without Medicare, we offer private pay options, and we provide a superbill at the time of service so patients can submit for potential out-of-network reimbursement on their own.

Patients using Medicare must have a physician order on file before an evaluation can be scheduled.

How to Refer a Patient

Complete the order form.

This form documents the requested evaluation and is required for any patient using Medicare.

Gather supporting documentation.

Please include with the order:

  • Most recent office visit note and/or history and physical (H&P)

  • Any relevant neuroimaging reports

  • Any relevant biomarker testing

  • Any records you feel would be valuable

Submit the order.

We handle the rest.

Our billing team will review the order for Medicare approval. Once approved, we will contact your patient directly to schedule. If a patient is not approved for services, we will notify your office as well as the patient, so you can determine next steps together.
